Current students Important Notices Health insurance! All students are required to carry health insurance for the duration of their enrollment. If you have not submitted proof of health insurance coverage, please e-mail your confirmation immediately to linda.hickman@notes.udayton.edu. Traveling outside the U.S. Please review the policy for travelling outside of the U.S. as an international student. Most students will require a "Travel Signature" before they can leave or re-enter the U.S. Read about travelling outside the U.S. >>      PDF document      Word document Update your address Students with a change of address must notify the DSO (International Student Services Coordinator) within ten days of an address change. Change of Address Form >> State ID cards State ID cards may be obtained from the Ohio Bureau of Motor Vehicles (BMV) for the cost of $8.50. Take your passport and visa, DS-2019, and I-94 card to the BMV to obtain an ID card. The BMV is located in Dayton at 451 West Third Street, Monday through Friday from 8:00 am to 4:30 pm. Contact them at (937) 225-4480 ext. 625. A social security number is NOT required to obtain an ID or a Driver's License in the state of Ohio. Many of the resources you encounter may state that the SSN is required to obtain a license or an ID card, but this only applies if the individual has been issued an SSN.
